Here’s a statistic that I think about every time I’m reviewing our product line: heel pressure ulcers account for roughly 30 percent of all hospital-acquired pressure injuries, and surgical patients are among the highest-risk because they literally cannot shift position during long procedures. The calcaneus is a sharply curved bony prominence with almost no subcutaneous fat pad protecting it — just a thin layer of skin and a bit of connective tissue over bone. After two hours of sustained pressure against a standard OR table surface, the tissue ischemia begins. After four hours, which is the length of a typical cardiac bypass or complex spine reconstruction, you’re looking at a Stage 1 or Stage 2 pressure ulcer that didn’t exist when the patient was wheeled into the OR. These injuries are classified as never events in many healthcare systems. They’re preventable. The AM-TW-015 is a small pad that addresses an outsized clinical problem.

At 16×9.5×5cm, the 015 is the most compact pad in our entire catalog, but the polymer gel formulation is identical to what goes into our full-size head and torso pads. The heel cup geometry cradles the calcaneus from three sides — posterior, medial, and lateral — suspending the bony prominence completely off the table surface. The critical engineering principle is suspension, not cushioning: foam heel protectors try to cushion the heel, but they bottom out after sustained loading and the calcaneus ends up resting on the table anyway. Our gel cup lifts the heel into a zero-pressure zone where the surrounding soft tissue — the Achilles tendon insertion, the medial and lateral malleolar soft tissue — carries the load while the calcaneus itself floats. The gel’s viscoelastic response absorbs micro-movements from table adjustments and Trendelenburg tilting without creating shear at the skin interface, which is actually the primary mechanism behind most device-related pressure injuries. No straps, no Velcro, no adhesive — the anti-slip base and contoured cup geometry hold the heel in place passively.

Unlike our limb positioning pads (AM-TW-012 concave pad, AM-TW-014 tunnel pad) that are designed for surgical access and limb stabilization, the 015 is purely a protective device. It’s for any surgical case longer than two hours where the patient’s heels are resting on the table surface — cardiac bypass, transplant procedures, complex spine cases, bariatric surgery, and prolonged laparoscopic procedures in steep Trendelenburg. Cheap foam heel protectors bottom out. The 015 doesn’t, because gel doesn’t memory-set and the suspension geometry is fundamentally different from a cushion.
